چکیده

Background The incidence of confirmed cow ́s milk protein (CMP) allergy in infancy is about 2-5% in developed countries. Until now, management strategies included only avoiding CMP intake, because no treatment was available. However, new therapeutic strategies have emerged in the last years. The most important is the induction of oral tolerance (IOT), with results which are promising, improving the quality of life, above all in those who can suffer a severe allergic reaction. Nevertheless, more studies about long-term effects are needed. We describe 4 cases of eosinophilic esophagitis (EoE) after IOT.

Debut of Crohn's disease in a child desensitised to cow’s milk

Specific oral immunotherapy (OIT) is a proven therapeutic option in children with persistent food allergy. Several cases of eosinophilic esophagitis have been reported during the long term follow-up, but not inflammatory bowel diseases. The role of food allergy in the pathogenesis of these diseases is unknown. We present an 11 year-old girl who developed cow’s milk allergy (CMA) at the age of 5...

Eosinophilic esophagitis: between reflux and allergy

Eosinophilic esophagitis (EoE) is a chronic/recurrent inflammatory disease of the esophagus characterized by the association between symptoms of esophageal dysfunction and a prominent eosinophilic infiltrate (>15/HPF) of the esophageal epithelium [1]. This condition mostly affects males and is increasingly being reported in all age groups, with a prevalence currently estimated at around 1:1000 ...

Non-IgE-mediated food allergy: a tolerance induction protocol

Background Cow’s milk proteins allergy (CMPA) is the most common food allergy in childhood, affecting up to 2.5% of infants. The immune mechanisms underlying non-IgE-mediated clinical presentations, which mostly affect the gastrointestinal system and are sometimes very severe, are not fully understood. Currently, specific oral tolerance induction (SOTI) is available as a specific treatment for ...

Specific oral tolerance induction (SOTI) to cow’s milk in an adult patient with anaphylaxis symptoms

Case report A 20-year-old man, with no personal history of atopic disease, reported at 19 years his first episode of anaphylaxis (conjunctival hyperemia, generalized pruritus, oropharyngeal obstruction sensation, dyspnea and dysphagia) 30 minutes after ingestion of 150ml of milk and bread with butter. Since then, he mentioned oropharyngeal soreness whenever he ingested milk. One year later, he ...

Persistence of CMP allergy in the 4 weeks before tolerance induction was based on positive skin and serum testing and a positive double-blind placebo-controlled food challenge to cow’s milk. Exclusion criteria included anaphylactic shoc...
